I would want to see more context, but I think "follow-up care" is best for a relatively amorphous offer, and "follow-up treatment" is appropriate when specific medication, counseling, etc., is offered.

extended access

I dont know enough about your context or audience.
But I suggest looking up 'continued access' or 'extended access program' which are terms used to describe allowing participants to continue taking the investigational product after the trial has ended. this may not be the lay term, though.
I think that followup care is something else (it is when the clinical trial program includes some monitoring of participants after the trial has ended, either by their own doctor or at the trial site).
