If you scroll down to the part that reads "DATOS DE LOS DECLARANTES Y TESTIGOS" there is a part that reads "Asi lo declaran:..... Como:......" followed by two drop-down menus containing the options "padre", "madre", "interesado" and "recomendado".

Here is another reference, from the Costa Rican Tribunal Supremo de Elecciones, entitled "Inscripción de Hechos Vitales y Actos Civiles". At the bottom of page 34, it reads:

Debe indicarse de manera clara y precisa, la calidad del declarante, si es como padre, madre, recomendado o interesado. También se anotarán sus nombres y apellidos legales, el número de la cédula o documento de identidad y quien declara debe firmar en el espacio respectivo. Si la persona firma con el conocido, se debe hacer la anotación correspondiente en el espacio para “observaciones”.

Unfortunately, I could find no other reference to "recomendado" in that document, but it would appear to mean something like a proxy or representative, as suggested in the discussion.
