Diagnosis: Correction of the cervical physiological lordosis with apparent pain syndrome irradiating to the right lower extremity, limitation of the head movements concomitant with pain syndrome. Lumbalgia of the lumbar region of the spine - pains irradiating to the left lower extremity. Lesion of the right radiocarpal joint, with impaired hand strength and apparent pain syndrome.
Roentgenography of the cervical and lumbar regions of the spine was carried out, roentgenography of the right radiocarpal joint.
Recommendations: Relaxed regime in the form of partial bed rest - 3 weeks, wearing orthesis, corset - 4 weeks, physiotherapeutic treatment course (adhesions, manual correction), kinesitherapy - N 15 (cervical, lumbar) nonsteroidal anti-inflammatory drugs (Arcoxia 90mg×1: three weeks, Voltaren Emulgel: use topically twice a day - 3 weeks), limit physical overwork, repeated visit in 21 days.

3. Հիմնադրամի և այլ անձանց իրավունքներն ու պարտականությունները

3.1. Հիմնադրամն իր կանոնադրության նպատակներին համապատասխան իրավունք ունի՝
1) անարգել տարածել տեղեկություններ իր գործունեության մասին.
2) հիմնադրել մամուլի և զանգվածային լրատվության այլ միջոցներ.
3) կատարել կանոնադրության փոփոխություններ.
4) պետական և տեղական ինքնակառավարման մարմիններից օրենքով սահմանված կարգով ստանալ տեղեկություններ, որոնք անհրաժեշտ են իր կանոնադրական նպատակներն իրականացնելու համար.
5) ստեղծել առանձնացված ստորաբաժանումներ (մասնաճյուղեր, ներկայացուցչություններ) և հիմնարկներ.
6) ստեղծել տնտեսական ընկերություններ կամ լինել նրանց մասնակից.
7) վերակազմակերպվել.
8) իրականացնել օրենքով չարգելված այլ գործունեություն:
Հիմնադրամի իրավունքները կարող են սահմանափակվել միայն օրենքով նախատեսված դեպքերում և օրենքով սահմանված կարգով:
3.2. Հիմնադրամը, Հայաստանի Հանրապետության օրենսդրությանը և իր կանոնադրությանը համապատասխան, կարող է անդամակցել միջազգային և օտարերկրյա ոչ պետական կազմակերպություններին:
3.3. Հիմնադրամը պարտավոր է՝
1) գործել Հայաստանի Հանրապետության Սահմանադրությանը, օրենքին և այլ օրենքներին ու իրավական ակտերին, ինչպես նաև սույն կանոնադրությանը համապատասխան.
2) օրենքով սահմանված կարգով վարել գործավարություն և հաշվապահական հաշվառում.

5) օրենքով և այլ իրավական ակտերով սահմանված կարգով վարել և ներկայացնել հաշվապահական և վիճակագրական հաշվետվություն.
6) իրականացնել օրենքով և այլ օրենքներով սահմանված այլ պարտականություններ:
3.4. Հիմնադրամին օգնություն ցուցաբերող անձինք (այդ թվում՝ Հիմնադրամի հիմնադիրը) իրավունք ունեն`
1) մասնակցել Հիմնադրամի գործունեության բոլոր ուղղություններին.
2) Հիմնադրամի կառավարման մարմինների կողմից, ինչպես նաև կնքված պայմանագրերով սահմանված պայմաններով Հիմնադրամի խնդիրներին ու նպատակներին համապատասխան ստանալ խորհրդատվական, փորձագիտական, միջնորդական, գիտատեխնիկական և այլ օգնություն.
3) Հիմնադրամի միջոցով ստեղծել և զարգացնել երկկողմ և բազմակողմ կապեր.
4) Հիմնադրամի կողմից նրա իրավունքների, իրավական և տնտեսական հնարավորու-թյունների շրջանակներում օգտվել սեփական շահերի պաշտպանությունից.
5) ցանկացած ժամանակ դադարեցնել իրենց մասնակցությունը Հիմնադրամի աշխատանք-ներին:
3.5. Հիմնադրամը վարում է իր գործունեությանը աջակցող անձանց ռեեստր:
3.6. Հիմնադրամին աջակցող անձինք պարտավոր են.
1) Հիմնադրամի ծրագրերի և միջոցառումների իրականացման ժամանակ գործել Հիմնադրա-մի կանոնադրության պահանջներին խիստ համապատասխան.
2) չտարածել Հիմնադրամի գործունեության մասին գաղտնի տեղեկություններ.
3) զերծ մնալ այնպիսի գործողություններից, որոնք կարող են վնաս պատճառել Հիմնադրամին:		Translation - English
3. Rights and obligations of the foundation and other persons

3.1. In compliance with its statutory objectives the Foundation shall have the right to:
1) disseminate information about its activities without restraint;
2) establish press and other means of mass media;
3) make amendments to the Charter;
4) receive information, necessary for accomplishment of its statutory objectives, from state and local self-government bodies in the procedure prescribed by Law;
5) create separate subdivisions (branches, representation offices) and institutions;
6) found economic companies or be a participant therein;
7) reorganize;
8) exercise other activities not prohibited by Law.
3.2. In compliance with the legislation of the Republic of Armenia and its Charter, the Foundation may be a member to international and foreign non-governmental organizations.
3.3. The Foundation shall be obliged to:
1) act in accordance with the Constitution of the Republic of Armenia, the Law and other laws and legal acts, as well as this Charter;
2) keep records and accounting in the manner stipulated by Law;
3) submit information and reports to state bodies in cases and in the manner stipulated by Law;
4) annually publish a report on its activities in accordance with the procedure and terms established by law;
5) lead and submit accounting and statistical reports in the manner prescribed by law and other legal acts;
6) fulfill other obligations envisaged by Law and other laws.
3.4. The persons providing aid to the Foundation (including the Founder of the Foundation) shall be obliged to:
1) participate in all spheres of activities of the Foundation;
2) receive consultative, experimental, brokerage, scientific-educational and other assistance in compliance with problems and objectives of the Foundation under conditions prescribed by the concluded contracts or stated by management bodies of the Foundation;
3) create and develop bilateral and multilateral relations through the Foundation;
4) enjoy the protection of personal interests by the Foundation within the frames of its rights, legal and economic opportunities;
5) at any time cease their participation in the activities of the Foundation.
3.5. The Foundation shall lead a register of persons assisting thereto.
3.6. The persons assisting the Foundation shall be obliged to:
1) while accomplishing projects and events, act in accordance with the requirements of the Charter of the Company;
2) not to disseminate confidential information about the activities of the Foundation;
3) avoid activities that may cause damage to the Foundation.

Never use solvents such as benzine or thinner to clean the TV.
These solvents may distort the cabinet or damage its finish.
If rubber or vinyl products remain in contact with the TV for a long time, a stain may result.

4. Կողմերի պատասխանատվությունը

4. Liability of the parties
4.1. In case the Lessee has not delivered back the Business or Area after the termination of the Contract or has delivered it by violating the term, then the Lessor shall have the right to demand the lease fee for all the period of delay. If such a payment does not fully indemnify the losses incurred by the Lessor, the latter may demand to indemnify the rest of it.

Likewise, dehiscences in the area of the origins of the renal arteries, and at the cephalic truncus too. Overall, a very large predominantly thrombosed aneurysm. Large renal cyst of approximately 5 cm, next to it a second smaller renal cyst. Further cysts in the parenchyma of the right kidney. Ectopic renal pelvis. Liver shape, position and size normal, Likewise normal appearance of the spleen. The pancreas appear rather small; only in the ventral portion the visibility of the pancreatic duct is reduced. In the upper portion of the liver, presumably in segments 7 to 8, reduced-density deposit․
No evidence of paraaortic, paracaval, iliac or inguinal abnormally enlarged LNs. Subcutaneously, air inclusion was noted in the abdominal skin on the right. TOP FOUR CAUSES OF DIARRHEA

1․ Intestinal Infections – Most of these are not serious, not treatable, and will resolve on their own with time:

Rotavirus – One of the most common causes of diarrhea, especially during late fall and winter months. It causes very foul smelling, watery, green or brown diarrhea that can persist for weeks. Fever and vomiting are common at the onset of the illness.

Other viruses – There are a variety of these, none of which are serious.

Bacteria – These include E. Coli, Salmonella, and several others. Vomiting and fever may be present at the onset. Blood in the diarrhea is a common finding with bacterial infections. Even these infections rarely require antibiotic treatment.

Parasites – There are a variety of these. They are usually caught from contaminated water (e.g. Giardia) or during travel to foreign countries. The telltale sign of a parasite is very watery diarrhea that lasts beyond two weeks.

CONTAGIOUS – These are all generally contagious as long as the diarrhea continues.

2․ Food Intolerance or Sensitivity – your child may simply be eating something that is difficult for his intestines to digest. These foods vary from person to person.

Lactose intolerance – This is the most common example of food intolerance. Symptoms include gassy pain, stomach cramps, nausea, and diarrhea after eating dairy products (which contain the sugar, lactose).

Sugar or chemical additives and coloring – These have been known to upset some little ones’ intestines.

3․ Food Allergy – There are many signs of food allergies, and mucusy, sometimes bloody, diarrhea is one of them. It is different from food intolerance in that the allergic symptoms affect several parts of the body – intestines, skin, respiratory.

Milk protein allergy – This is an example of one of the most common food allergies. Your child may start to develop diarrhea, rash on the face, body, and diaper area, runny nose, cough, or wheezing. In this case, the child is allergic to the protein in dairy products, where as with the dairy intolerance above, his intestines have a difficult time digesting the milk sugar, lactose.

4․ Normal Intestinal Variations – As discussed above, your child may simply be going through a normal phase of loose stools. It may be related to a change in diet, but doesn’t always mean there is an allergy or intolerance.		Translation - Armenian

4. Управление обществом

4.1. Органами управления Обществом являются общее собрание участников Общества и исполнительный орган Общества - Директор.
4.2. Высшим органом управления Обществом является общее собрание участников Общества, которое имеет право на окончательное решение любого вопроса, касающегося к управлению и деятельности Общества.
4.3. Очередное общее собрание участников Общества созывается Директором Общества один раз в год, не ранее чем через два месяца и не позднее чем через четыре месяцев после окончания финансового года. Общество обязано ежегодно созывать очередное общее собрание участников для утверждения годовых результатов деятельности Общества.
4.4. Руководство текущей деятельностью Общества осуществляется Директором, который избирается общим собранием участников Общества.
Директор вправе без доверенности совершать сделки от имени Общества, если сумма сделки не превышает 25% стоимости чистых активов Общества.

ВИДЕОГАСТРОСКОП ШТОРЦ свободно проведен за глоточное кольцо, пищевод свободно проходим. В дистальной части пищевода определяется гастроэзофагеальный анастомоз. Анастомоз правильной формы, проходимость хорошая. Слизистая в зоне анастомоза гиперемированна, отечная, отмечаются много эрозий неправильной формы. Поверхность эрозий покрыто сероватым налетем. В просвете желудка много мутнозеленоватой жидкости, складки мозговидные, эластичные, слизистая розовая, отечная. В антральной части имеются единичные эрозии диаметром 0,2см. Привратник и луковица ДПК правильной формы. Слизистая луковицы розовая, несколько отечная.

ЗАКЛЮЧЕНИЕ: Картина выраженного эрозивного эзофагогастрального анастомоза, эрозивного воспаления дистальной части слизистой пищевода. Поверхностный гастродуоденит с эрозиями в антральной части желудка.

STORZ VIDEO GASTROSCOPE has been freely conducted through the pharyngeal lymphoid ring; the esophagus is patent. In the distal part of the esophagus, a gastroesophageal anastomosis is detected. The anastomosis is of a regular shape, the patency is proper. The mucosa within the region of the anastomosis is hyperemic, edematous, multiple erosions of irregular shape are detected. The surface of the stomach is covered with grayish incrustation. In the lumen of the stomach there are multiple cloudy greenish liquids, the lines are cerebriform, elastic, the mucos is pink, edematous. In the antral part there are single erosions with diameter about 0,2cm. The pylorus and spindles of WPC are of a regular shape. The mucosal spindles are pink, a bit edematous.

CONCLUSION: An image of an expressed erosive gastroesophageal anastomosis, erosive inflammation of the distal part of the mucosa of the esophagus. Superficial gastroduodenitis with erosions in the antral part of the stomach.

The Akhurian gorge has great aesthetic characteristics. In certain places the depth of the Akhuryan gorge reaches 50-60 m, the walls of which compose vertical basalt cliffs sometimes flooded with river water. In some places the gorge is multilayered /has rising grounds/, composed of lava slabs. Though vegetation inside lava layers and cliff cracks is not widespread, it is distinguished in its diversity of species. Petrophilic plants coexist with dry grains and mainly with rosehip, spirae, cotoneaster and ribes bushes. There are plants belonging to Crassuceae family. There are early spring plants, for example, Muscari sp., Fritillaria caucasica, Gagea sp.. Most part of the gorged landscape is composed of gulches adjacent to the gorge, in some part of which there are fast and everflowing streams. A stream like this near Jradzor village creates a marvellous waterfall. In the gorged landscape one may encounter clump of trees and separate thick-stemmed trees, typically of Salix and Populus types. The animal world tamed to the gorges is also abundant, particularly there is a wide variety of invertebrates. There are mountain birds, waterbirds, and predators that inhabit on the cliffy area of the gorge.

The intrahepatic ducts and the choledochus are not dilated. The pancreatic gland is without any focal formations. The pancreatic duct is not dilated. The spleen is not enlarged, without foci. Kidneys are with clear circuits, normally positioned, without focal formations. PCS /pelvicalyceal system/ and the ureters are not dilated, timely intake of contrast preparation in the cavitary system. The adrenal glands are without formations. Lymph nodes of the abdominal cavity are not enlarged. No ascites. Bone destruction foci are not detected in the area of scanning.
