Introduction: This Guide to Local Production of WHO-recommended Handrub Formulations is separated into two discrete but interrelated sections:

Part A provides a practical guide for use at the pharmacy bench during the actual preparation of the formulation. Users may want to display the material on the wall of the production unit.

Part B summarizes some essential background technical information and is taken from WHO Guidelines on Hand Hygiene in Health Care (2009). Within Part B the user has access to important safety and cost information and supplementary material relating to dispensers and distribution.

PART A: GUIDE TO LOCAL PRODUCTION
Part A is intended to guide a local producer in the actual preparation of the formulation.

Materials required (small volume production)
• 10-litre glass or plastic bottles with screw-threaded stoppers, or
• 50-litre plastic tanks (preferably in polypropylene or high density polyethylene, translucent so as to see the liquid level), or
• Stainless steel tanks with a capacity of 80–100 litres (for mixing without overflowing)
• Wooden, plastic or metal paddles for mixing
• Measuring cylinders and measuring jugs
• Plastic or metal funnel

REAGENTS FOR FORMULATION 1:
• Ethanol 96%
• Hydrogen peroxide 3%
• Glycerol 98%
• Sterile distilled or boiled cold water

REAGENTS FOR FORMULATION 2:
• Isopropyl alcohol 99.8%
• Hydrogen peroxide 3%
• Glycerol 98%
• Sterile distilled or boiled cold water

NOTE
• Glycerol: used as humectant, but other emollients may be used for skin care, provided that they are cheap, widely available and miscible in water and alcohol and do not add to toxicity, or promote allergy.
• Hydrogen peroxide: used to inactivate contaminating bacterial spores in the solution and is not an active substance for hand antisepsis.
• Any further additive to both formulations should be clearly labelled and be non-toxic in case of accidental ingestion.
• A colorant may be added to allow differentiation from other fluids, but should not add to toxicity, promote allergy, or interfere with antimicrobial properties. The addition of perfumes or dyes is not recommended due to risk of allergic reactions.
• 100 ml plastic bottles with leak-proof tops
• 500 ml glass or plastic bottles with screw tops
• An alcoholometer: the temperature scale is at the bottom
and the ethanol concentration (percentage v/v) at the top

METHOD: 10-LITRE PREPARATIONS
These can be prepared in 10-litre glass or plastic bottles with
screw-threaded stoppers.

Step by step preparation:
1. The alcohol for the formula to be used is poured into the large bottle or tank up to the graduated mark.
2. Hydrogen peroxide is added using the measuring cylinder.
3. Glycerol is added using a measuring cylinder. As glycerol is very viscous and sticks to the wall of the measuring cylinder, it should be rinsed with some sterile distilled or cold boiled water and then emptied into the bottle/tank.
4. The bottle/tank is then topped up to the 10-litre mark with sterile distilled or cold boiled water.
5. The lid or the screw cap is placed on the tank/bottle as soon as possible after preparation, in order to prevent evaporation.
6. The solution is mixed by shaking gently where appropriate or by using a paddle.
7. Immediately divide up the solution into its final containers (e.g. 500 or 100 ml plastic bottles), and place the bottles in quarantine for 72 hours before use. This allows time for any spores present in the alcohol or the new/re-used bottles to be destroyed.

Pendahuluan: Panduan untuk Produksi Formulasi Handrub Lokal Rekomendasi WHO ini dibagi menjadi dua bagian terpisah, tetapi saling terkait:

Bagian A memberikan panduan praktis untuk pengerjaan di atas meja kerja apotek dalam penyiapan formulasi yang sebenarnya. Pengguna mungkin perlu memajang materi ini di dinding unit produksi.

Bagian B merangkum informasi teknis latar belakang penting yang diambil dari Pedoman Kebersihan Tangan dalam Perawatan Kesehatan WHO (2009). Di Bagian B, pengguna dapat mengakses informasi keamanan dan biaya yang penting serta bahan pelengkap yang berkaitan dengan dispenser dan distribusi.

BAGIAN A: PANDUAN UNTUK PRODUKSI LOKAL
Bagian A dimaksudkan untuk memandu pemroduksi lokal dalam penyiapan formulasi yang sebenarnya.

Bahan yang diperlukan (produksi volume kecil)
• Botol kaca atau plastik berkapasitas 10 liter dengan penutup berulir, atau
• Tangki plastik 50 liter (sebaiknya terbuat dari polipropilena atau polietilena densitas tinggi dan transparan sehingga ketinggian cairan dapat dilihat), atau
• Tangki baja tahan karat berkapasitas 80-100 liter (agar dapat mencampur tanpa terjadi luapan)
• Pengaduk kayu, plastik, atau logam untuk mencampur
• Gelas ukur dan gelas takar
• Corong plastik atau logam
• Botol plastik 100 ml dengan tutup tahan bocor
• Botol kaca atau plastik 500 ml dengan tutup berulir
• Alkoholmeter: skala suhu di bagian bawah dan konsentrasi etanol (persen v/v) di bagian atas

REAGEN UNTUK FORMULASI 1:
• Etanol 96%
• Hidrogen peroksida 3%
• Gliserol 98%
• Air suling steril atau air matang dingin

REAGEN UNTUK FORMULASI 2:
• Isopropil alkohol 99,8%

CATATAN
• Gliserol: Humektan atau emolien lain dapat digunakan untuk perawatan kulit, asalkan bahan tersebut tidak mahal, tersedia lokal, tercampurkan dalam air dan alkohol, tidak toksik, atau menyebabkan alergi.
• Hidrogen peroksida: digunakan untuk menginaktivasi kontaminasi spora bakteri dalam larutan dan bukan zat aktif untuk antisepsis tangan.
• Zat tambahan lain dalam dua formulasi harus diberi label dengan jelas dan tidak toksik jika terjadi penelanan tidak disengaja.
• Zat pewarna boleh ditambahkan untuk membedakan dari cairan lain, tetapi tidak boleh menambah toksisitas, memicu alergi, atau mengganggu sifat antimikroba. Penambahan pewangi tidak dianjurkan karena risiko reaksi alergi.

METODE: SEDIAAN 10 LITER
Sediaan ini dapat dibuat dalam botol kaca atau plastik 10 liter dengan tutup berulir.

Persiapan langkah demi langkah:
1. Alkohol yang akan digunakan untuk formula dituang ke dalam botol besar atau galon sampai tanda batas.
2. Hidrogen peroksida ditambahkan menggunakan gelas ukur.
3. Gliserol ditambahkan menggunakan gelas ukur. Karena gliserol sangat kental dan menempel di dinding gelas ukur, gelas harus dibilas dengan air suling steril atau air matang dingin lalu hasil bilasan dituang ke dalam botol/galon.
4. Lalu, botol/galon diisi dengan air suling steril atau air matang dingin sampai tanda 10 liter.
5. Pasang tutup pada tangki/galon sesegera mungkin setelah penyiapan guna mencegah evaporasi.
6. Larutan dicampur dengan menggoyang perlahan, jika perlu, atau dengan mengaduk menggunakan sendok pengaduk.
7. Segera bagi larutan ke dalam wadah akhir (msl. botol plastik 500 atau 100 ml), dan tempatkan botol dalam karantina 72 jam sebelum digunakan. Kurun waktu ini untuk memusnahkan spora yang ada dalam alkohol atau botol baru/pakai-ulang.

I have an MSc degree in Molecular Mechanism of Disease (Molecular Biology) from Radboud University, Netherlands, BSc degree in Pharmacy, Professional Pharmacist Diploma, and ASc degree in Computer Science. Regarding credentials, I am a full member of Association of Indonesian Translators and I am MTPE certified (from SDL MTPE Certification).

From 2010 to 2015, I worked for a university textbook publisher, EGC Medical Publisher, in Indonesia. I have followed a formal training for editors. I was involved in the translation, editing, and proofreading of university textbooks in the fields of pharmaceutical, medical, biology, etc.
