Source text - English Alternative Syk-specific, Bruton’s tyrosine kinase–specific, and phosphoinositide 3-kinase–specific inhibitors are presently under development, showing promising preclinical activity in CLL models. Many of these new agents are undergoing evaluation in phase I clinical trials. Additional clinical trials are needed to establish the role of all these new agents, primarily to determine whether monotherapy or combination therapy with other targeted agents or with chemotherapy in previously untreated and refractory/relapsed patients with CLL is the optimal treatment strategy. More mature data are needed in terms of tolerability, safety, and emerging mechanisms of resistance for all these new agents. Nevertheless, we can imagine a future where targeted agents are routinely used and chemotherapy is reserved for use only in limited circumstances.
Other active compounds in the pipeline target cellular mechanisms of apoptosis to shift the balance from cell cycle arrest and DNA repair to irreparable DNA damage and apoptosis. Some agents in the pipeline include flavopiridol and GDC-0199/ABT-199.
Flavopiridol, also known as alvocidib, is a cyclin-dependent kinase inhibitor that has been shown to be effective in relapsed, high-risk patients with CLL.[Byrd 2007; Lin 2008; Lin 2009] Despite ongoing concern about this agent’s history of hyperacute tumor lysis syndrome, recent data showed improved safety with a new dosing schedule as well as promising effects. A phase II study of 117 relapsed patients demonstrated a 48% ORR.[Byrd 2007; Lin 2008] Another phase II study in patients with relapsed CLL showed a 53% ORR, including patients with del(11q) or del(17p), with flavopiridol therapy.[Lin 2009] Additionally, reducing the number of weekly treatments per cycle from 4 to 3 as well as the addition of prophylactic dexamethasone mitigated cytokine release syndrome (P ≤ .01).
