Между L5-S1 имеется парамедианная протрузия диска справа.
Снижение диаметра позвоночного канала отчасти обусловлена уже отмеченными ранее изменениями в дисках, и отчасти врожденным укорочением ножек.

Any patients who are suspected, probable or confirmed cases and present for care at a healthcare facility should be placed directly into individual rooms with the door kept closed. Note that some Remote Sites may not have a room and an isolated area should be used instead. Healthcare personnel interacting with the patients should follow the infection control guidance in this document.
Once a person is suspected of having H1N1, the following infection control practices must be instituted immediately:
• Standard Precautions
• Airborne Precautions
• Droplet Precautions
• Contact Precautions

Все пациенты с подозрением на заболевание, с вероятным или подтвержденным диагнозом, обратившиеся за помощью в медицинское учреждение, должны быть незамедлительно помещены в отдельные палаты, дверь в которые должна держаться закрытой. Необходимо учитывать, что не во всех клиниках на отдаленных локациях есть возможность оборудовать отдельные комнаты, в таких случаях следует использовать любое изолированное помещение. Source text - Russian Больной К.Д. поступил в отделение 20.08.05 в 3.10, куда был доставлен бригадой Скорой Помощи. Со слов доставившего врача больной был обнаружен в горящей квартире пожарными в бессознательном состоянии.
При поступлении состояние тяжелое , что обусловлено угнетением ц.н.с. вплоть до комы, по шкале Глазго 6 -7 б., дыхательной недостаточностью в виде экспираторной одышки- ЧДД до 32-34 в мин. Был госпитализирован в реанимационную палату, где после интубации трахеи был переведен на ИВЛ в режиме умеренной гипервентиляции. На R-снимках при поступлении диффузное затемнение легких с обеих сторон по типу отёка легких, на контрольном снимке в 10.30 явления отёка легких уменьшились. Больной осмотрен пульмонологом. Больному проведена ФБС, выявлен термический ожог ТБД 2 ст. Проведена санация раствором фурациллина.
Осмотрен неврологом: гипоксическая энцефалопатия. Отёк головного мозга. Эпиреакция.
Диагноз: Острое бытовое отравление продуктами горения тяжелой степени. Термический ожог верхних дыхательных путей 2 степени. Отек легких. Острая дыхательная недостаточность 3 ст. Острая энцефалопатия смешанного генеза (токсическая, постгипоксическая). Кома 3 ст. Отек головного мозга. Эпиреакция
Больному проводилась медикаментозная защита мозга: антибактериальная терапия цефтриаксоном, сибазон по 10 мг в.в, тиопентал Na в.в 200 мг в час. Начато введение преднизолона по 60 мг в.в
Лабораторно: кщс от 4.20- рH- 7.11, рСО2- 53.9, р О2- 26.6, ВЕ- 11.7; кщс от 5.20- рН-7.31, рСО2- 36.7, рО2-22.1, ВЕ- 5.3. Для коррекции ацидоза вводилась сода всего 500 мл 4 % р-ра. Биохимия: глюкоза- 13.3, о.белок- 72, билирубин – 10, креатинин- 107.
Состояние больного остаётся тяжёлым, проводится ИВЛ, сохраняется энцефалопатия. Для дальнейшего лечения переводится в университетскую клинику г. Франкфурт-на-Майне. Транспортирован в аэропорт в сопровождении доктора [C. С.] в условиях реанимационной бригады.
Translation - English Case Report #21517
Pt C… D, had been admitted into the Department at 3:10 a.m. on 08/20/05, where he had been transported by ambulance service. According to the words of the ambulance doctor, the patient was found unconscious in a burning apartment by fire fighters.
On admission the Pt’s condition is serious, due to CNS functions suppression and coma (GCS 6-7 points), respiratory failure in the mode of expiratory shortness of breath, RR – 32-34 per 1 minute. The Pt had been hospitalized into ICU, intubated and put on ventilation in moderate hyperventilation mode. Chest X-ray showed a diffuse opacity in both lungs, similar to pulmonary edema; control chest X-ray at 10:30 a.m. signs of pulmonary edema have decreased. A consultation of a pulmonologist was obtained. The Pt underwent Fibrobronchoscopy, which showed thermal burn of thracheobroncheal tree, 2nd degree. A lavage was performed with Furacilin solution.
Consultation of a neurologist: hypoxical encephalopathy; cerebral edema, epireaction.
Diagnosis: acute intoxication of combustion products in domestic setting, severe degree. Thermal burn of the upper respiratory tract, second-degree. Pulmonary edema. Acute respiratory failure, III degree. Acute encephalopathy of mixed origin (toxic and posthypoxic). Coma III, cerebral edema, Epireaction.
The Pt had received brain protection management: anti-bacterial treatment with Ceftriaxon, Diazepam 10 mg I/V, Thiopental Sodium 200 mg I/V per 1 hour. Prednisone 60 mg I/V had been started.
Lab tests: ABB at 4.20 a.m.- рH- 7.11, рСО2- 53.9, рО2- 26.6, ВЕ- 11.7; ABB at 5.20 a.m.- рН-7.31, рСО2- 36.7, рО2-22.1, ВЕ- 5.3. For acidosis correction sodium bicarbonate 500 ml 4% sln infusion was performed. Biochemical analysis: Blood glucose - 13.3 mmol/l, protein- 72, bilirubin – 10, kreatinin- 107.
The Pt’s condition remains to be serious, ALV is continued, encephalopathy persists. The Pt is going to be transferred to the University clinic of Frankfurt-on-Mein for further treatment. The Pt had been transported to the airport with the escort of [S.S.] MD in the setting of intensive care team.
