Cambios en la tendencia sobre la mortalidad por enfermedad pulmonar obstructiva crónica: ¿realidad o ficción?
Debido a su elevada prevalencia, morbimortalidad y costes asociados, la enfermedad pulmonar obstructiva crónica (EPOC) constituye un grave problema sanitario en la actualidad. Los efectos relacionados con la exposición tabáquica acumulada, el envejecimiento paulatino de la población, la disminución en el infradiagnóstico y la influencia de los nuevos tratamientos y mejores condiciones de vida son factores que podrían producir cambios en la prevalencia y la mortalidad de la EPOC. Se espera que tanto la prevalencia como la mortalidad alcancen su máximo en las próximas 2-3 décadas. Sin embargo, diversos estudios de cohortes nacionales, europeos y norteamericanos sobre tendencias de mortalidad por EPOC han generado cierta incertidumbre sobre si realmente esta enfermedad se está estabilizando o incluso está disminuyendo. Especialmente relevante es el reciente trabajo de López-Campos et al., que ha analizado el patrón de mortalidad por EPOC de 27 países europeos (incluyendo España) de la serie temporal transcurrida entre 1994 y 2010. Los resultados indican que para la gran mayoría de países existe una tendencia constante y decreciente en la mortalidad en varones, siendo el descenso mucho más débil en las mujeres, lo que nos hace ver el futuro con algo más de optimismo.
Translation - English Changes in Chronic Obstructive Pulmonary Disease Mortality Trends: Fact or Fiction?
Because of its high prevalence, morbidity and mortality and associated costs, chronic obstructive pulmonary disease (COPD) is today a major health problem. The cumulative effects of exposure to tobacco smoke, progressive population ageing, fewer cases of under-diagnosis and the effect of new treatments and better living conditions could change COPD prevalence and mortality rates, both of which are expected to peak in the next 20 to 30 years. However, several Spanish, European and US cohort studies in COPD mortality trends have questioned whether prevalence is actually stabilising or even falling. Of particular relevance is the work of López-Campos et al. in analysing COPD mortality patterns in 27 European countries (including Spain) between 1994 and 2010. The study shows a steady downward trend in mortality among men, although far less so among women, in most of the countries studied, suggesting a far more optimistic view of the future of the disease.
